Distinctive characteristics of this form:

area above the message boxes is now divided into five distinct sections;
Telegraph Charges - for 16 words - have been moved back to under the TELEGRAM heading;
space but no box for the Date Stamp;
the main distinctions amongst the three formats are:
4A: details of the Sender are preceeded by FROM;
4B: no FROM for sender and one schedule number (at left);
4C: no FROM for sender and two schedule numbers (one on each side of the form).

Transmission form used to make a copy of the submitted message.

Characteristics:

details of sender begin with "From" with two lines following;
instructions about telegraphing the details of the sender are in two sentences in a block of text in the lower right corner;
the schedule number is at the same level as the second address line.
